Home visits are contingent on provider approval and must be within a 20 mile radius from the clinic
Cancellation Policy
1. To cancel or reschedule, please contact us 24 hours in advance. If an appointment is canceled with less than 24 hours' notice, no refund will be given. By booking this appointment, you are agreeing to this cancellation policy. 2. No Insurance Billing: • The Patient acknowledges that this plan does not involve insurance billing, and the Patient is fully responsible for payments for all services received. 3. We do not accept Medicare and Medicaid : Powerful Health and Wellness, LLC will not bill Medicare and Medicaid and choses to opt out from their billing. The patient is solely responsible for all payments. 4. Medical Emergencies: • This plan does not cover medical emergencies. In the event of a medical emergency, the Patient agrees to call 911 or go to the nearest emergency department. 5. By Signing or paying and checking policy checkbox on website you agree to all additional policies provided on the website policy page. Hard copies can be provided upon request.
